Key terms (12)
Therapeutic communication
Purposeful communication directed at the patient's needs rather than the nurse's or the ward's, distinguished by its purpose and not by its tone, as opposed to social conversation that builds rapport and collegial communication that is structured such as SBAR.
Termination phase
The final phase of the therapeutic relationship after orientation and working, which is the phase most often skipped and is closed deliberately by summarising what was achieved, saying what happens next and who takes over, and saying goodbye.
False reassurance
A non-therapeutic reply such as "Don't worry, everything will be fine" that dismisses the fear, promises what nobody can promise and teaches the patient not to raise it again, and is the most commonly tested wrong answer.
Empathy
Understanding and naming the patient's feeling from his own position, such as "This must be frightening", which keeps the focus on him, unlike sympathy, which is feeling sorry from the nurse's own position.
A six-step sequence for breaking bad news (Setting, Perception, Invitation, Knowledge, Emotions, Strategy and summary), in which the nurse's role is usually to prepare the setting, be present and do the work afterwards, while the diagnosis is given by the treating physician.
Shared decision-making
An approach that puts the reasonable options, including doing nothing, on the table with their benefits, harms and uncertainties so that the patient's values decide between them, which differs from informed consent, where the patient only accepts or refuses a plan the team has chosen.
Health literacy
The ability to obtain, understand and act on health information; low health literacy is common, invisible and not reliably predicted by general education, so plain language is used with every patient.
Teach-backالاسترجاع التعليمي
A check in which the patient is asked to say what he will do, framed as a test of the nurse's own explanation, because "do you understand?" and nodding are not evidence of understanding.
Domains of learning
The cognitive domain (knowing, taught by explanation and evaluated by stating it back), the affective domain (attitudes and feelings, approached by discussion) and the psychomotor domain (doing, taught by demonstration and evaluated only by return demonstration).
Stages of change
The sequence of precontemplation, contemplation, preparation, action and maintenance, with relapse as part of the cycle, in which the response must fit the patient's actual stage, such as raising awareness in precontemplation rather than handing over a plan.
Levels of prevention
Primary prevention acts before the disease exists, secondary prevention detects it early in someone who has it without knowing (so screening a well person is secondary), and tertiary prevention limits damage in established disease, as in rehabilitation.
A structured communication format in which A is Assessment (the nurse's own clinical interpretation, not an address or allergies) and R is Recommendation (a specific request with a time frame), with ISBAR adding Identify using two patient identifiers.

3.1 · Communication as a Clinical Skill

Communication failure is one of the largest contributors to preventable harm in hospitals, which is why effective communication between caregivers is one of the six International Patient Safety Goals. It is a clinical skill that is taught, assessed and improved — not a personality trait some nurses happen to have.

It has measurable effects. Patients who understand their plan take their medicines more reliably, return to hospital less often, and complain less — and most complaints about clinical care turn out, on examination, to be complaints about communication rather than about the treatment given.

1
SOCIAL conversation
Builds rapport — the weather, his family, the football. It is not filler; it is what makes the next two possible.
2
THERAPEUTIC communication
PURPOSEFUL and directed at the PATIENT’S needs — not yours, and not the ward’s.
3
COLLEGIAL communication
Structured, precise and directed at a clinical decision — SBAR, handover, escalation.
4
And they are NOT interchangeable
Social warmth in place of a therapeutic conversation leaves the fear unaddressed; therapeutic openness in a handover wastes the window — knowing which register you are in is the skill.
⭐
Key point
Therapeutic communication is distinguished by purpose, not by tone. A warm chat that serves the nurse's comfort rather than the patient's need is social, however kind it sounds.

3.2 · The Therapeutic Relationship

1
ORIENTATION
Introduce yourself and your ROLE, establish what the patient wants from the contact, and set the boundaries and the expected duration.
2
WORKING
The problem is explored and worked on — the longest phase, and where the therapeutic techniques live.
3
TERMINATION
The phase most often SKIPPED — and it matters most to patients who have been in hospital a long time.
4
Why skipping it harms
Ending abruptly — transferring or discharging without a closing conversation — leaves the patient feeling ABANDONED, and it undoes some of what the relationship achieved.
5
So close it deliberately
Summarise what was achieved, say what happens next and who takes over, and say goodbye — which takes two minutes and is remembered.

The relationship is UNEQUAL IN POWER and THERAPEUTIC rather than SOCIAL — which is what makes boundaries necessary. The line falls at secrecy, off-duty contact, gifts beyond a token, and self-disclosure that serves you rather than him.

Termination is the phase most often skipped and it matters most to patients who have been in hospital a long time. Ending abruptly — transferring or discharging without a closing conversation — leaves the relationship unfinished and is experienced as abandonment even when the care was excellent. Some texts add a pre-interaction phase before the first meeting (preparing, reviewing the record, checking one's own feelings), and some use Peplau's four-phase wording (orientation, identification, exploitation, resolution); this chapter teaches the three-phase form.

The relationship is unequal in power and therapeutic rather than social, which is what makes boundaries necessary. Chapter 01, section 1.17 sets out where the line falls: secrecy, off-duty contact, gifts of value, and treating one patient as special.

3.3 · Non-Verbal Communication

Most of what a patient reads from you is NOT IN YOUR WORDS: posture, distance, eye contact, facial expression, touch, tone — and how much you appear to be IN A HURRY.

SIT DOWN at the bedsideReliably makes patients report a LONGER conversation than standing — the same number of minutes, felt differentlyالجلوس بجانب سرير المريض: يجعل المرضى يُبلّغون باطّراد عن حديث أطول مقارنة بالوقوف — والمدة بالدقائق نفسها لكنها تُدرَك بشكل مختلف
Hand on the door handlesays ‘be quick’ whatever your words say
Eye level, not standing overreduces the power gap you cannot otherwise remove
•Two rules about non-verbal communication◆
When words and BEHAVIOUR disagree, the patient believes the BEHAVIOURAnd so should you. A patient who says he is fine while GRIPPING THE SHEET, AVOIDING YOUR EYES and answering in SHORT SENTENCES is telling you something — and the right response is to name what you see, gently, and wait.
Non-verbal norms are CULTURALLY VARIABLEDirect eye contact, personal distance, and touch between men and women carry DIFFERENT meanings in different communities — and what reads as warm attentiveness to one patient reads as intrusive to another. Watch the response and adjust, rather than applying one style to everyone.

When words and behaviour disagree, the patient believes the behaviour. A patient who says he is fine while gripping the sheet, avoiding your eyes and answering in short sentences is telling you something, and naming the mismatch gently — "you say you're fine, but you seem uncomfortable" — is itself a therapeutic technique.

Non-verbal norms are culturally variable. Direct eye contact, personal distance, and touch between men and women carry different meanings in different communities, and what reads as warm attentiveness to one patient reads as intrusive to another.

⭐
Key point
When the verbal and the non-verbal conflict, explore rather than accept the words. "I'm fine" delivered with clenched hands is a cue, not a conclusion.

3.4 · Therapeutic Techniques

1
OPEN-ENDED questions
‘Tell me about the pain’ rather than ‘is the pain bad?’ — invites more than a yes.
2
REFLECTION
Returns the patient’s own FEELING to him — ‘you sound frightened.’
3
RESTATING
Repeats his CONTENT in your words — which shows you were listening and lets him correct you.
4
CLARIFYING
‘When you say dizzy, do you mean the room spins, or that you feel faint?’ — two different symptoms.
5
PURPOSEFUL SILENCE
Gives him room to think and to find words for something difficult.
6
OFFERING SELF
‘I’ll sit with you for a while’ — signals availability WITHOUT demanding disclosure.
1
SUMMARISING
Draws the thread together and checks you have it right — and gives him a chance to add what he left out.
2
FOCUSING
Brings a wandering account back to what matters — ‘you mentioned the chest pain — can we come back to that?’
3
EXPLORING
Goes deeper into something raised — rather than noting it and moving on.
4
PRESENTING REALITY
Gently corrects a misperception WITHOUT arguing — ‘I don’t hear that, but I can see it frightens you.’

Purposeful silence gives the patient room to think and to find words for something difficult. Offering self — "I'll sit with you for a while" — signals availability without demanding disclosure. Summarising closes a conversation by pulling the threads together and checking you heard correctly.

Focusing brings a wandering account back to what matters: "you mentioned the chest pain — can we come back to that?" Exploring goes deeper into something raised. Presenting reality gently corrects a misperception without arguing: "I don't hear the voices, but I understand that you do."

In an exam transcript, the therapeutic option almost always keeps the conversation open and hands control back to the patient. The wrong options close it, redirect it, or hand control to the nurse.

3.5 · Non-Therapeutic Responses

❌FALSE REASSURANCE — the most tested error
  • ‘Don’t worry, everything will be fine’«
  • It is NOT kindness
  • It DISMISSES the fear
  • It PROMISES what nobody can promise
  • And it teaches the patient not to raise it again
✅What to say instead
  • Name what you see: ‘you sound worried about tomorrow’
  • Then STOP and let him fill the space
  • Give honest information where you have itتُقدَّم معلومة صادقة حيثما توفّرت
  • And say what you DO know: ‘I will be here, and I will stay with you’ويُقال ما هو معروف فعلاً: «سأكون هنا، وسأبقى معك»
  • Which is a promise you can actually keep

MINIMISING and STEREOTYPED responses‘Others have it worse’ INVALIDATES; ‘everything happens for a reason’ is a phrase, not a response — and both are heard as ‘stop talking’.
And APPROVING or DISAPPROVING‘That’s good’ and ‘you shouldn’t feel that way’ both make YOUR judgement the standard — which is exactly what a patient stops telling the truth to.

"Why" questions demand a justification and put the patient on the defensive — ask "what happened?" or "tell me about it" instead. Giving advice takes the decision away from the patient. Changing the subject tells him the topic is unwelcome. Approving or disapproving makes your regard conditional on his choices.

Defensiveness — defending the ward when a patient complains — ends the complaint without addressing it. Minimising ("others have it worse") invalidates. Stereotyped responses ("everything happens for a reason") signal that you are not listening. And false agreement to avoid an awkward moment stores up a bigger problem later.

False reassurance is the commonest wrong answer in communication questions. If an option promises a good outcome, it is almost certainly wrong.

3.6 · Listening, Empathy and Sympathy

ACTIVE LISTENING is VISIBLE WORK: facing the patient, unhurried posture, not interrupting, following HIS direction rather than your checklist, and reflecting back what you heard. It is not silence while you wait for your turn to speak, and the difference is obvious from the outside.

  • ‘I feel so sorry for you.’«أشعر بأسف شديد من أجلك»

  • He then has to manage your distress as well as his ownفيضطر المريض حينها إلى التعامل مع ضيق مقدّم الرعاية بالإضافة إلى ضيقه هو
  • Which is why it is the wrong answer on the exam

Empathy is understanding and naming the patient's feeling from his position: "this must be frightening." Sympathy is feeling sorry from your own position: "I feel so sorry for you." Empathy keeps the focus on him and keeps you useful; sympathy moves the focus to your feelings and often leads to reassurance you cannot back.

Empathy names the patient's feeling. Sympathy announces yours. In an option list, the empathic reply is about him; the sympathetic one is about you.

SILENCE is an INTERVENTION, not a gap. After bad news, after a difficult disclosure, or when a patient is trying to find words, the useful response is TO STAY AND SAY NOTHING. Nurses fill silence because it is uncomfortable for THEM — and in filling it, they take away the moment the patient needed to speak.

3.8 · Anger, Demands and Distress

Anger is usually a SURFACE for something underneath — fear, pain, powerlessness, or a GENUINE FAILURE in the care given. The last of those is the one worth checking first.

1
Stay CALM and keep your voice LOW
Volume escalates; a low voice de-escalates — and he will match you.
2
Move somewhere PRIVATE
Anger performed in front of an audience is harder to step back from. But stay in sight of colleagues, with the door open and help within reach, rather than alone with him.
3
LISTEN without interrupting
All the way to the end — which is usually shorter than you expect.
4
ACKNOWLEDGE the feeling and the FACTS
‘You waited two hours and nobody told you why. I would be angry too.’
5
Find out WHAT HE WANTS
Which is often smaller than the anger suggests — an explanation, an apology, or one thing fixed.
6

Two things are never the answer: defending the ward before you have understood the complaint, and promising something you cannot deliver to end the confrontation. Both feel like resolution in the moment and both make the next conversation worse.

Safety comes first when anger escalates towards threat: keep the exit behind you, do not stand over the person or corner him, keep an arm's length, call for help early, and disengage rather than win. A patient who is threatening is a safety event, and it is reported.

3.10 · Death, Dying and the Bereaved Family

•Use PLAIN words◆
‘DIED’ and ‘DEATH’ are clearer than the alternativesNot ‘passed’, ‘lost’, ‘gone’ or ‘no longer with us’. Euphemism is a common source of families NOT UNDERSTANDING WHAT HAS ACTUALLY HAPPENED — particularly across a language barrier, and particularly with children.«توفّي» و«الوفاة» أوضح من البدائل — لا «انتقل» ولا «فقدناه» ولا «رحل» ولا «لم يعد بيننا»؛ فالتلطيف سبب شائع لعدم فهم الأسر ما حدث فعلاً، ولا سيما عند وجود حاجز لغوي ومع الأطفال.
News of a death is NOT given by telephoneWhen a family must be called, the message is that the patient’s CONDITION HAS CHANGED SERIOUSLY and that they should COME IN — with someone driving them if possible, because people who have just been told drive badly.
1
With a DYING patient — keep speaking to him
Hearing is widely held to persist LATE, so continue to speak to him, introduce yourself, and say what you are about to do.
2
Answer honestly when asked HOW LONG
An honest ‘I don’t know, but I think it is short’ is better than a number you cannot stand behind — families plan around what you say.
3
With the BEREAVED family — PRESENCE outweighs words
Say you are SORRY, use the PERSON’S NAME, allow silence and tears, and RESIST filling the space with explanation.
4
Avoid the stock phrases
‘He’s at peace now’, ‘at least he didn’t suffer’, ‘you must be strong’ — all of which are heard as instructions to stop feeling.
5
And attend to what happens next
Somewhere private, time with the body, the rites they need, and clear practical information about what must be done — which is what they will remember.

News of a death is not given by telephone. When a family must be called, the message is that the patient's condition has changed seriously and that they should come in, with someone driving them if possible — and the death is told in person, in a private room, by someone who can stay. Being told on the phone leaves a family alone with it and travelling in distress. If a relative asks directly whether the patient has died, do not lie: say that this cannot be discussed properly over the telephone and that someone will meet them on arrival; if the question is pressed, answer plainly and stay on the line.

With a dying patient: hearing is widely held to persist late, so continue to speak to him, introduce yourself, and say what you are about to do. Answer honestly when asked how long — an honest "I don't know, but the changes we're seeing suggest it may be short" is better received than a confident number or an evasion. Do not promise there will be no pain; say what you will do about it.

With a bereaved family, presence outweighs words. Say you are sorry, use the person's name, allow silence and tears, and resist filling the space with explanation. Stock phrases — "he's at peace now", "at least he didn't suffer", "you must be strong" — are heard as dismissal. Religious and cultural practices around washing, shrouding and timing are part of the care, as Chapter 01 sets out.

For a patient with dementia: approach from the front, use his name, one short instruction at a time, allow long pauses for processing, and do not test him with questions about date or place that only expose the deficit. When he says something factually wrong, respond to the feeling behind it rather than correcting the fact — arguing about reality raises distress and changes nothing.

3
Give him TIME — far more than feels comfortable
The commonest failures are GIVING UP AFTER TWO ATTEMPTS, and GUESSING ALOUD until he stops correcting you.
4
And do not let the call bell be his only route
Check on him without being called — because frustration and fear are the dominant experiences of a patient who cannot speak, and being unable to summon help is the heart of it.

Never talk across a patient as though he is absent. Handover, prognosis discussions and social conversation at the bedside are all heard, and being discussed in the third person while lying there is one of the experiences intensive care survivors describe most consistently.

For an intubated or tracheostomised patient who is awake, build a route out. Ask questions that need only yes or no, and agree a signal — a blink, a hand squeeze, a nod. Offer an alphabet board, a picture or symbol board, a writing pad if the hands allow, or a communication application. Establish a reliable call signal before he needs it, and give him more time than feels comfortable.

Frustration and fear are the dominant experiences of a patient who cannot speak, and the commonest failures are giving up after two attempts, guessing aloud until he stops correcting you, and letting the family answer for him. Record in the notes which method worked, so the next nurse does not begin again from nothing.

3.15 · Language Barriers and Interpreters

A QUALIFIED INTERPRETER is required whenever the patient cannot communicate reliably in the language being used — and ALWAYS for consent, diagnosis, discharge teaching, and anything with legal weight.

Working with an interpreter: speak to the patient, not to the interpreter, keep eye contact with the patient, use short segments and pause, use plain language without idiom, and check understanding with teach-back through the interpreter. Allow roughly twice the usual time, and record in the notes that an interpreter was used and who it was.

3.16 · Communicating in Saudi Practice

1
Patients and staff often do not share a first language
So bilingual capability and interpreter access are ROUTINE CLINICAL NEEDS, not exceptions — and planned for, not improvised.
2
MODESTY and gender preferences
Requested rather than assumed, and ARRANGED rather than promised — including who may be present during an examination.
3
Prayer, fasting and religious observance
Planned around, where it is safe — and asked about at admission rather than discovered at a bad moment.
4
And a highly MULTINATIONAL workforce
Expectations about hierarchy, directness and speaking up differ substantially — so expectations are stated explicitly, and a nod is checked rather than assumed to mean agreement.
•Family-centred communication — and the two questions that make it work◆ التواصل المتمحور حول الأسرة — والسؤالان اللذان يجعلانه ينجح

Requests to WITHHOLD a diagnosis are commonAnd handled the same way everywhere: you may NOT LIE to a patient who ASKS — but he MAY CHOOSE NOT TO BE TOLD. That distinction resolves almost every case.
So EXPLORE why the family is askingIt is nearly always protection, not control — and a conversation about what he already suspects usually resolves it, because most patients already know.

Family-centred communication is the norm and it does not remove the patient's autonomy. The workable approach is to ask the patient directly, early and privately, who he wants present in conversations and who he wants informed — then follow what he says rather than what the family assumes. That one question prevents most of the conflicts that follow.

Requests to withhold a diagnosis from the patient are common and are handled the same way everywhere: you may not lie to a patient who asks, but he may choose not to be told. Explore why the family is asking, find out from the patient how much he wants to know, and bring the treating physician into it — Chapter 01, section 1.1 covers the veracity rule.

3.17 · Patient-Centred Care and Shared Decisions

PATIENT-CENTRED CARE means care organised around THIS PATIENT’S values, preferences and expressed needs — rather than around the ward’s routine. In practice it shows up in small things: when meals arrive, when he washes, who is allowed to stay, and whether anyone asked.

✍️INFORMED CONSENT

  • WITH their benefits, harms and uncertainties
  • Including the option of DOING NOTHING
  • And the patient’s VALUES decide between themوقيم المريض هي ما يحسم بينها
  • They are NOT the same thing — which is the examined point

Shared decision-making is not the same as informed consent. Consent asks the patient to accept or refuse a plan the team has chosen. Shared decision-making puts the reasonable options on the table with their benefits and harms, elicits what matters to the patient, and reaches a decision together. It is the right approach wherever there is genuine equipoise between options.

  • Which removes the shame and gets you an honest answer

Plain language: short sentences, everyday words, one idea at a time, the most important point first, and no jargon without translating it — "high blood pressure" rather than hypertension, "water tablet" rather than diuretic. Aim for three key points per session, not everything you know, and support words with a picture or a demonstration.

Confirm with teach-back, covered in Chapter 02: ask the patient to say what he will do, and frame it as a check on your own explaining rather than a test of him — "I want to be sure I explained this well; can you tell me how you'll take it?"

🚩
Watch out
A patient nodding and saying yes is the least reliable evidence of understanding you can collect. Nodding is politeness; teach-back is evidence.

3.19 · Readiness, Barriers and the Domains of Learning

Before teaching anything, assess READINESS. A patient in severe pain, acutely breathless, sedated, frightened or newly bereaved CANNOT LEARN — and teaching delivered into that state is wasted, however well it is done.

1
COGNITIVE — KNOWING
Taught by EXPLANATION. Evaluated by the patient STATING IT BACK.
2
AFFECTIVE — ATTITUDE and FEELING
Taught by DISCUSSION, exploring beliefs and addressing fear. Evaluated by what he SAYS about it and how he behaves — and it is the domain that decides whether the cognitive learning is ever used.
3
PSYCHOMOTOR — DOING
Taught by DEMONSTRATION and practice. Evaluated by RETURN DEMONSTRATION — he does it, you watch. Telling him how to use an inhaler is not teaching him to use an inhaler.

Other barriers to check: pain, fatigue, sensory impairment, language, low literacy, anxiety, denial, cultural mismatch between the teaching and the patient’s life, and simply the wrong time of day. Most are removable, and removing one is usually faster than teaching harder.

Three domains of learning, each needing a different method and a different evaluation. Cognitive — knowing: taught by explanation, evaluated by the patient stating it back. Affective — attitudes and feelings: approached by discussion and exploring concerns, evaluated by what he expresses. Psychomotor — doing: taught by demonstration, evaluated only by return demonstration.

⭐
Key point
If the skill is a doing skill, nothing except a return demonstration counts as evaluation. A patient who can describe an inhaler perfectly may still use it wrongly.

1
They are SELF-DIRECTED
And want a say in WHAT is taught — so you agree the agenda rather than deliver one.
2
They bring EXPERIENCE
Which must be BUILT ON rather than contradicted — correcting a belief head-on usually entrenches it.
3
They are PROBLEM-CENTRED, not subject-centred
They want to solve THIS problem, not learn the topic.
4
They need to know WHY
Relevance is the condition for attention — give the reason before the instruction.
5
And motivation is mostly INTERNAL
Driven by what matters in their own life — the grandchild, the job, the pilgrimage — not by your target.
1
AGREE what he wants to get from it
First — which takes thirty seconds and changes everything after it.

2

More than three and none of them is retained.

3

4
Get a RETURN demonstration or TEACH-BACK
Which is the only way you know it worked.
5
Give something WRITTEN to take away
In his language, key action first.
6
And arrange WHEN it will be reinforced
Once is not teaching — and the second contact is where it sticks.

A workable teaching session: agree what the patient wants to get from it, teach three points at most, use plain language and a demonstration, get a return demonstration or teach-back, give a written summary at a readable level, and schedule a short review rather than a single long session. Short and repeated beats long and once.

Match the response to the stage. Handing a smoking-cessation plan to someone in precontemplation produces resistance; the useful move there is to raise awareness and invite him to consider it. Detailed practical planning belongs in preparation, and relapse-prevention work belongs in maintenance.

Motivational interviewing works with ambivalence instead of arguing against it: open questions, affirmations, reflective listening and summaries. Arguing for change reliably produces argument against it — the patient defends the behaviour aloud and persuades himself in the process.

Meet the patient where he is. The commonest error in health promotion questions is offering an action-stage intervention to a precontemplation-stage patient.

3.22 · Teaching Groups and Communities

Group teaching is NOT individual teaching repeated to more people. It is efficient, and it adds PEER LEARNING — patients accept from someone living with the same condition what they resist from a professional.

1
Plan BACKWARDS from what people must be able to DO
At the end — not forwards from what you want to cover.
2
Write TWO or THREE behavioural objectives
Observable verbs only.
3
Match METHOD to DOMAIN
Explanation for knowledge; DISCUSSION for attitude; DEMONSTRATION and practice for skill.
4
Keep the group small enough to speak
And arrange the seating so people face each other, not the front.
5
Plan for the DOMINANT talker and the SILENT one
Both need managing, and neither manages itself.

  • Return demonstration, a short quiz, a stated plan
  • Its purpose is to judge the session, not to rescue itغرضه الحكم على الجلسة، لا إنقاذها

Community health education extends the same thinking beyond the ward: know WHO the audience is and WHAT THEY ALREADY BELIEVE; work through the settings and people they already TRUST — the mosque, the school, the family elder; match the message to the culture and the language; and evaluate by BEHAVIOUR, not by attendance.

Plan a session backwards from what people must be able to do at the end. Write two or three behavioural objectives, choose methods that match the domain — explanation for knowledge, discussion for attitudes, demonstration and practice for skills — allow time for questions, and finish with a summary and a written take-home at a plain reading level.

Evaluate at two levels. Formative evaluation happens during the session and lets you adjust — questions back, a show of hands, watching who is lost. Summative evaluation happens at the end and measures whether the objectives were met: a return demonstration for a skill, a short quiz for knowledge, and where possible an outcome measure weeks later, which is the only evidence that anything changed.

3.23 · Health Promotion and Levels of Prevention

1
PRIMARY — before the disease exists
Immunisation, health education, seat belts, safe water, smoking prevention. Nobody has the disease.
2
SECONDARY — detect it EARLY
In someone who HAS it but does not know — screening: mammography, blood pressure checks, cervical screening (Pap smear), blood glucose screening for undiagnosed diabetes.
3
TERTIARY — limit the damage
In established disease — rehabilitation, preventing complications (for example diabetic foot care), preventing another stroke.
4
The distinction that catches people
SCREENING A WELL PERSON IS SECONDARY, NOT PRIMARY — because the disease may ALREADY BE PRESENT. You are looking for it early, not preventing it. This is asked directly.

HEALTH PROMOTION is BROADER than prevention: it is enabling people to increase CONTROL over their health, through education, environment and policy TOGETHER. Nursing contributes at every level — the immunisation given, the screening offered opportunistically, the rehabilitation supported, and the policy argued for.

The distinction that catches people: screening a well person is secondary prevention, not primary, because the disease may already be present — you are looking for it early rather than preventing it. And rehabilitation after a myocardial infarction is tertiary, even though it also prevents a second event.

Say plainly that you need a decision, and then ESCALATE — to the nurse in charge and up the chain. Documenting each step as you go.

Conflict handled well is a NORMAL part of team work. Address it EARLY and PRIVATELY; describe the BEHAVIOUR and its EFFECT rather than labelling the person; listen to the other account BEFORE responding; look for the shared goal — which is almost always the patient; and agree what will happen differently next time, because a conversation that ends without that changes nothing.

When a concern is not acted on, escalate using a graded approach: state the observation, state your concern, propose a solution, and if it is still not addressed, say plainly that you need a decision because the patient is at risk. Then go up the chain of command and document each step, as Chapter 01, section 1.18 sets out.

Conflict handled well is a normal part of team work. Address it early and privately, describe the behaviour and its effect rather than labelling the person, listen to the other account before responding, and separate the problem from the personality. Collaboration produces the best outcomes; avoidance works only for trivia; competing is for emergencies where someone must decide now.

A is ASSESSMENT — your interpretationNOT address, NOT allergies, and not a repeat of the observations. R is RECOMMENDATION — a specific request with a time frameNOT a summary and NOT ‘I just wanted to let you know’.
Some services use ISBARAdding IDENTIFY at the front — your name, your role, your unit, and the patient’s identity confirmed with TWO IDENTIFIERS. It exists because calls in which nobody established who was speaking about whom have caused real harm.

And the rule that runs through all of it: anything clinical taken VERBALLY is WRITTEN DOWN and READ BACK AT THE TIME — orders and critical results alike, while the other person is still on the line.

Two letters are routinely got wrong. A is Assessment — your clinical interpretation, the sentence beginning "I think" or "I'm concerned that" — and not address, allergies or anything else. And R is Recommendation, an explicit request with a timeframe. "I just wanted to let you know" is not a recommendation, and a call that ends without one usually has to be made again.

Some services use ISBAR, adding Identify at the front — your name, your role, your unit, and the patient's identity confirmed with two identifiers. It exists because calls in which nobody establishes who is speaking about whom are a recurring source of error.

Anything clinical taken verbally is written down and read back at the time — orders and critical results alike. Chapter 02 covers shift handover, transfer handover and verbal orders in full; the point here is that structure is what replaces the non-verbal information a telephone strips out.
