Key terms (12)
Nursing process (ADPIE)العملية التمريضية (ADPIE)
The five steps of assessment, diagnosis, planning, implementation and evaluation, which form a cycle rather than a checklist because evaluation feeds back into assessment.
Cue and inference
A cue is what the nurse observed (such as pacing, wringing the hands and a pulse of 104), whereas an inference is the conclusion drawn from it (such as "appears anxious"), so the cues are the part that belongs in the record.
Nursing diagnosis
A statement that names the patient's response rather than the disease, written as problem, related factor and evidence, while a risk diagnosis has only two parts (problem and risk factors) because there is no evidence yet.
Actual before potential
A prioritising rule by which a problem happening now outranks a risk being managed, so new confusion with a respiratory rate of 30 outranks three patients who are merely at risk of falling, infection or pressure injury.
A structured communication format of Situation, Background, Assessment and Recommendation, in which the recommendation is an action with a time frame (for example, review within ten minutes) that turns a report into a request.
Read-back of verbal orders
The safeguard for verbal or telephone orders, permitted only in emergencies or when the prescriber cannot attend: write it down, read it back aloud in full (drug, dose, route and frequency), confirm while the prescriber is still on the line, then record it and obtain countersignature.
Charting by exception
A documentation format that records only what falls outside a predefined standard, with the standard itself serving as the record of normal, and which depends entirely on those standards being explicit, known and followed.
Medication reconciliation
Comparing what the patient was actually taking with what is now prescribed at admission, transfer and discharge, designed to catch an omitted home medicine, a hospital medicine continued in error, a duplicate, or a wrong dose or frequency.
Teach-backالاسترجاع التعليمي
Asking the patient to explain the plan back in his own words, which is the only way to know discharge teaching has landed; asking "do you understand?" is not teach-back.
Disaster triage
Triage used when casualties exceed resources, aiming at the greatest number of survivors so that the sickest may not go first, with categories red (immediate), yellow (delayed), green (minor) and black (expectant), repeated as patients change.
Two patient identifiers
Two pieces of patient-specific information, such as full name and medical record number, used to identify a patient correctly; the room or bed number is never an identifier because it belongs to the bed, not the person.
Just culture
An approach that distinguishes human error (consoled and the system fixed), at-risk behaviour (coached and the incentive removed) and reckless behaviour (the only one sanctioned), so that reporting and learning are encouraged.

2.1 · The Nursing Process: ADPIE٢٫١ · العملية التمريضية: ADPIE

1
A — ASSESSMENT
Collect the data. Subjective and objective, from the patient first.
2
D — DIAGNOSIS
Name the patient’s RESPONSE — not the disease.
3
P — PLANNING
Set measurable, patient-centred GOALS and choose interventions.
4
I — IMPLEMENTATION
Carry out the care, and document it.
5
E — EVALUATION
Did the goal happen? Measured against the outcome you wrote, not against effort.
•It is a CYCLE, not a checklist
Evaluation feeds straight back into ASSESSMENTA goal that is MET closes a problem and frees attention for the next one.
A goal that is NOT met sends you backTo look for what you MISSED — not to repeat the same plan harder.
And in practice the steps overlapYou are assessing while you implement — the sequence is a discipline for thinking, not a queue.
Why it existsIt gives nursing a structure that can be TAUGHT, AUDITED and DEFENDED — and it is what turns doing tasks into practising a profession.

It is a cycle, not a checklist. Evaluation feeds straight back into assessment: a goal that is met closes a problem and frees attention for the next one, and a goal that is not met sends you back to look for what you missed. In practice the steps overlap — you are assessing while you implement, and evaluating while you assess.

Assessment · Diagnosis · Planning · Implementation · Evaluation. When an exam question asks what the nurse should do first and no one is dying, the answer is almost always the assessment step.

2.2 · Assessment: Types and Sources

1
COMPREHENSIVE
At admission, covering every system — the baseline everything else is compared against.
2
FOCUSED
ONE problem in depth, when something new or changing appears.
3
EMERGENCY
Airway, breathing, circulation — done while you are treating, not before.
4
ONGOING
Repeated to track a trend — which is where deterioration is actually detected.
👤PRIMARY source
  • The PATIENT — always
  • His own account of his own experience
  • Ranked above every other source
  • Even when the family disagrees with himحتى لو خالفته الأسرة الرأي
👥SECONDARY sources
  • Family, other staff, existing records
  • Useful and used constantly
  • But ranked BELOW the patient’s own account
  • The ranking matters most when the accounts DISAGREE

Data that does not FIT the clinical picture is VALIDATED before it is acted on. A blood pressure of 200/110 in a comfortable, chatting patient is REPEATED with the correct cuff before anyone gives a drug or makes a call — and a wrong cuff size is a common reason for a false reading.

The patient is always the primary source. Family, other staff and existing records are secondary sources — useful, used constantly, but ranked below the patient's own account. The distinction matters most when the accounts disagree.

Key point
Assessment comes before intervention every single time, with one exception: an immediate threat to airway, breathing or circulation is acted on while you assess, not after.

•A CUE is what you saw; an INFERENCE is what you concluded
‘Appears anxious’ is an INFERENCEIt is a conclusion recorded as if it were an observation — and nobody afterwards can tell what you actually saw.«يبدو قلقاً» استنتاج — خلاصة سُجّلت وكأنها ملاحظة، ولا يستطيع أحد بعد ذلك معرفة ما شوهد فعلاً.
This is the CUE SET that supports it‘Pacing the bay, wringing his hands, pulse 104, says he cannot stop thinking about tomorrow’ — which is what belongs in the record.
So chart the CUES and let the inference followThe inference may be revised; the cues remain true — which is why the record holds one and not the other.

A cue is what you saw; an inference is what you concluded from it. "Appears anxious" is an inference. "Pacing the bay, wringing his hands, pulse 104, says he cannot stop thinking about tomorrow" is the cue set that supports it. Charting the inference alone hides the evidence and cannot be checked by anyone else.

🚩
Watch out
Charting a conclusion in place of an observation is a common documentation weakness in nursing notes, and one a reviewer is likely to challenge.

2.4 · Writing the Nursing Diagnosis

1
PROBLEM
Names the human RESPONSE — what the patient is experiencing.
2
RELATED FACTOR
Names the CAUSE you can act on.
3
EVIDENCE
The signs and symptoms that show it is happening.
4
Put together
‘Impaired skin integrity RELATED TO prolonged immobility, EVIDENCED BY a stage 2 sacral ulcer.’
5
A RISK diagnosis has only TWO parts
Problem and RISK FACTORS — because there is NO evidence yet. ‘Risk for falls related to sedation and an unfamiliar environment.’ You cannot write ‘evidenced by’ for something that has not happened.
•Four ways it goes wrong
Writing a MEDICAL description instead of a response‘Pneumonia’ is not a nursing diagnosis; ‘impaired gas exchange’ is.
Making it CIRCULARWhere the related factor just repeats the evidence — ‘pain related to pain’ in longer words.
Naming a NURSING ACTION as the cause‘related to lack of turning’ — which describes a care failure, not a patient factor.
And BLAMING staff or the patientInstead of naming a factor — ‘related to non-compliance’ is a judgement, not a diagnosis.

Four ways it goes wrong: writing a medical description instead of a response; making the statement circular, where the related factor repeats the evidence; naming a nursing action as the cause; and blaming staff or the patient instead of naming a clinical factor.

Read your diagnosis back and ask: does the related factor tell me something I can actually act on? If changing it would not change the care, it is not an aetiology.

2.5 · Nursing, Medical and Collaborative Problems

🩺MEDICAL diagnosis
  • Names the DISEASE
  • Pneumonia; ischaemic stroke; atrial fibrillation
  • Made by the PHYSICIAN
  • Does NOT change through the admission
👩‍⚕️NURSING diagnosis
  • Names the patient’s RESPONSE to itيُسمّي استجابة المريض له
  • Impaired gas exchange; activity intolerance; anxiety
  • Made by the NURSE
  • CHANGES as the patient changes — which is why it is reviewed

A COLLABORATIVE problem sits between the two: a physiological complication the nurse MONITORS to detect early and MANAGES JOINTLY with medical orders — falling potassium on a diuretic, rising intracranial pressure, post-operative haemorrhage. The distinguishing feature: the nurse cannot prevent or treat it independently, but she is the one who DETECTS it, which is why monitoring is the intervention.

⭐
Key point
Two patients with the same medical diagnosis will usually have different nursing diagnoses. That difference is the whole argument for individualised care planning.

1
SPECIFIC
One thing, clearly stated.
2
MEASURABLE
With a number or an observable action.
3
ACHIEVABLE and REALISTIC
For THIS patient, with these resources.
4
TIME-BOUND
By when — without which evaluation has no trigger.
5
And avoid the unmeasurable verbs
UNDERSTAND, KNOW, APPRECIATE and FEEL cannot be measured. Replace them with something you can SEE him do: STATE, DEMONSTRATE, WALK, LIST, SELF-ADMINISTER.

SHORT-term goals are reached in hours to days and step towards LONG-term goals that may run past discharge. Both are set WITH the patient — a goal he did not agree to is a goal he will not work towards. And a standardised care plan is a STARTING POINT that must then be INDIVIDUALISED — for this patient, with these resources, and this home to go back to.

Every goal should be specific, measurable, achievable, realistic and time-bound. Verbs like understand, know, appreciate and feel cannot be measured — replace them with something you can see the patient do: state, demonstrate, walk, list, self-administer.

2.7 · Prioritising: The Three Rules

1
RULE 1 — PHYSIOLOGY first
Airway before Breathing, Breathing before Circulation, Circulation before Disability. A narrowing airway beats a falling blood pressure, which beats a low glucose, which beats a wound that is oozing.
2
RULE 2 — ACTUAL before POTENTIAL
A problem HAPPENING NOW outranks a risk being managed. New confusion with a respiratory rate of 30 outranks three patients who are merely AT RISK of falling, infection or pressure damage.
3
RULE 3 — UNEXPECTED before EXPECTED
Serosanguineous drainage on day one after surgery is EXPECTED; a rigid, silent abdomen on day four is NOT. Learn the normal course of each condition — that is what lets you recognise a departure from it.
•MASLOW sits underneath all threeيقع هرم ماسلو تحت القواعد الثلاث جميعاً
PHYSIOLOGICALAir, water, food, elimination, sleep, pain relief — the base, and it wins.
SAFETYPhysical safety, and freedom from fear.
BELONGING, then ESTEEM, then SELF-ACTUALISATIONIn that order.
And it settles the four-call-bells questionBREATHLESSNESS before a request to move beds, and BOTH before a question about visiting hours — which is exactly how the exam frames it.

Rule two — actual before potential. A problem that is happening now outranks a risk that is being managed. New confusion with a respiratory rate of 30 outranks three patients who are merely at risk of falling, infection or pressure injury.

Rule three — unexpected before expected. Serosanguineous drainage on day one after surgery is expected; a rigid, silent abdomen on day four is not. Learn the normal course of each condition, because that is what lets you recognise the deviation.

Maslow sits underneath all three: physiological needs, then safety, then belonging, then esteem, then self-actualisation. It settles the four-call-bells question — breathlessness before a request to move beds, and both before a question about the ward round.

⭐
Key point
When two rules point in different directions, physiology wins. An actual problem that is not life-threatening still yields to an airway that is.

2.8 · Deciding Who to See First٢٫٨ · تحديد من يجب رؤيته أولاً

Exam questions and real shifts both hand you FOUR PATIENTS AT ONCE. Work through them in the SAME ORDER every time.

1
1 — Is anyone’s AIRWAY, BREATHING or CIRCULATION threatened?
If yes, that is the answer. Stop here.
2
2 — Is anyone UNSTABLE or NEWLY CHANGED?
A change from an hour ago outranks a bad number that has been stable all week.
3
3 — Is any finding UNEXPECTED for the condition and the day?
Which is why you must know the normal course.
4
4 — Then the ACTUAL problems before the RISKS
And the time-critical before the merely urgent.
5
5 — And only then, comfort and information
Which still get done — just not first.
STRIDOR or a change in VOICEairway — go nowصرير أو تغيّر صوت — مجرى الهواء: التحرّك فوراً
Falling conscious level with VOMITINGairway at risk
THROAT TIGHTNESS after neck surgeryhaematoma compressing the airway
A RIGID abdomenperitonitis
NEW CONFUSION with tachypnoeasepsis or hypoxia until proved otherwise
CHEST PAIN, or a sudden severe headachego, and take help with youألم صدر أو صداع شديد مفاجئ — التحرّك فوراً واصطحاب المساعدة

Some findings should move you immediately whatever else is happening: stridor or a change in voice, a falling conscious level with vomiting, throat tightness after neck surgery, a rigid abdomen, new confusion with tachypnoea, chest pain with sweating, and unilateral calf pain with swelling and warmth.

1
INDEPENDENT
The nurse initiates them herself — repositioning, mouth care, teaching, reassurance. No order needed.
2
DEPENDENT
Require a prescriber’s ORDER — giving a drug, adjusting an infusion.
3
INTERDEPENDENT / collaborative
Carried out WITH other disciplines — a physiotherapy programme, a dietetic plan, a discharge package.
•Every intervention begins with a REASSESSMENT

Every intervention begins with a reassessment. A plan is written for the patient's expected condition; if he is now pale, dizzy and hypotensive, the planned walk is suspended and the team informed. It is not carried out at a reduced distance.

Evaluation compares the patient’s CURRENT status against the OUTCOME CRITERIA WRITTEN FOR HIM — never against other patients, and never against how busy the shift was.

GOAL METclose the problem, free attention for the next
PARTLY METcontinue, and adjust
NOT METgo back through the cycle
1
Was the ASSESSMENT incomplete?
Did you miss data that changes the picture?
2
Was the DIAGNOSIS wrong?
You may have named the wrong response.
3
Was the GOAL unrealistic?
For this patient, in this time frame.
4
Was the INTERVENTION ineffective?
Or the wrong one for the cause.
5
Or was it simply NOT CARRIED OUT?
Which is the honest answer more often than the others — and it is a different problem entirely.

The TIMING of evaluation follows the INTERVENTION, not the shift. Reassess pain when the analgesic should be working, around its expected PEAK effect, not at the end of the shift; reassess an antipyretic within an hour; reassess a fluid bolus by its effect on blood pressure and urine output — because an evaluation done at the wrong time evaluates nothing.

A goal that is not met, or only partly met, sends you back through the cycle. Was the assessment incomplete? Was the diagnosis wrong? Was the goal unrealistic, or the intervention ineffective, or simply not carried out? Extending the target date without answering one of those questions changes nothing.

A goal that is fully met is closed and assessment starts again. The process does not end while the patient is still in your care.

2.11 · Organising the Shift

1
Identify WHO must be seen first
Before you touch anything.
2
Identify what is TIME-CRITICAL
Pre-meal insulin, antibiotic times, pre-operative preparation, time-limited observations — these anchor the shift and everything else moves around them.
3
Group what can be done in ONE visit
Observations, medication, a turn and a drink together — which halves the walking and doubles the time at the bedside.
4
Identify what can be DELEGATED
And to whom, with clear reporting parameters.
5
And RE-PLAN whenever something changes
A deteriorating patient REORDERS THE WHOLE SHIFT — and the nurse who keeps working the original list because it is written down is the nurse who misses the deterioration.

Re-plan whenever something changes. A deteriorating patient reorders the whole shift, and the nurse who keeps working the original list because it is written down is the nurse who misses the deterioration.

2.12 · Handover and SBAR٢٫١٢ · التسليم وأسلوب SBAR

1
S — SITUATION
Who this is and what is happening RIGHT NOW. Name, ward, and the problem in one sentence.
2
B — BACKGROUND
The admitting problem, the day after surgery, the relevant comorbidities.
3
A — ASSESSMENT
What YOU think is going on — with the observations that support it. You are allowed to have a view.
4
R — RECOMMENDATION
What you want them to DO, and BY WHEN.
✅A RECOMMENDATIONتوصية
  • ‘I would like you to review him WITHIN TEN MINUTES’«
  • An action and a time frame
  • It turns a report into a REQUEST
  • And it creates a record of what you asked forويُنشئ سجلّاً بما طُلب
❌NOT a recommendation
  • ‘I just wanted to let you know’«فقط أردت إعلامك»
  • The part most often left outالجزء الأكثر إغفالاً
  • It transfers information but not responsibility
  • And when nothing happens, nothing was actually asked

Shift handover is safest AT THE BEDSIDE, in a structured format: the patient can hear and CORRECT what is said, both nurses can SEE him, and omissions are caught. Corridor handovers break confidentiality and lose detail — and the detail they lose is usually the one nobody thought to write down.

The recommendation is the part most often left out, and it is the part that turns a report into a request. "I would like you to review him within ten minutes" is a recommendation; "I just wanted to let you know" is not.

2.13 · Verbal and Telephone Orders

Verbal and telephone orders are permitted IN EMERGENCIES and when the prescriber CANNOT ATTEND — not for convenience.

1
WRITE IT DOWN
As he says it — not from memory afterwards.
2
READ IT BACK
Aloud, in full — drug, dose, route, frequency.
3
Get CONFIRMATION while he is STILL ON THE LINE
This is what catches the misheard dose — and it is the whole point of the sequence.

4
Enter it in the record
With the date, time, prescriber’s name and your signature.

5

•An order that is AMBIGUOUS, INCOMPLETE, or uses a PROHIBITED abbreviation is clarified BEFORE anything is givenالأمر الغامض أو الناقص أو المكتوب باختصار ممنوع يُوضَّح قبل إعطاء أيّ شيء
‘U’ for unitsMisread as a ZERO or a FOUR — turning 4U into 40 or 44.
‘QD’ for dailyMisread as QID — four times the intended dose.«QD» يومياً — تُقرأ QID: أربعة أضعاف الجرعة المقصودة.
A NAKED decimal — ‘.5 mg’Read as 5 mg — a TENFOLD overdose. Always write a LEADING ZERO: 0.5 mg.
And a TRAILING zero — ‘5.0 mg’Read as 50 mg if the point is missed. Never write a trailing zero.

The order is then entered in the record and countersigned by the prescriber within the period the policy allows. Chapter 01, section 1.10 covers the accountability side of this.

An order that is ambiguous, incomplete, or written with a prohibited abbreviation is clarified before anything is given. "U" for units is misread as a zero or a four; "QD" is misread as "QID"; a dose written as ".5 mg" is read as "5 mg" when the decimal point is missed. Never assume the intended dose, however obvious it appears.

The commonest error is skipping straight from a single cue to an action. A respiratory rate of 30 is a cue; it becomes useful only when set beside the temperature, the conscious level, the day after surgery and the trend over the last six hours. One value is data; a pattern is information.

Ask two questions of every abnormal finding: is this expected for this patient at this stage, and is it changing. A temperature of 37.8 °C on the first evening after surgery is expected and static. The same reading on day five, rising, with a tender wound, is a different finding entirely.

2.17 · Frameworks for Organising Assessment

1
HEAD-TO-TOE
Works DOWN the body in order, and is hard to leave gaps in — which is why it suits admission and shift assessments.
2
BODY SYSTEMS
Respiratory, cardiovascular, neurological and so on — suits a patient with a known problem in one system.
3
GORDON’S functional health patterns
ELEVEN patterns of living — health perception, nutrition, elimination, activity, sleep, cognition, self-perception, roles, sexuality, coping, and values and beliefs. Its strength: it surfaces what a physical survey misses.
4
ABCDE — the EMERGENCY framework
Airway, Breathing, Circulation, Disability, Exposure — used whenever a patient is acutely unwell, in that order, and treating what you find BEFORE moving on.
5
Which to choose
The framework follows the QUESTION — a stable admission gets head-to-toe; a deteriorating patient gets ABCDE; a complex social discharge gets Gordon’s.

Gordon's functional health patterns organise assessment around eleven patterns of living — health perception, nutrition, elimination, activity, sleep, cognition, self-perception, roles, sexuality, coping, and values and beliefs. Its value is that it surfaces the problems a systems review misses: coping, roles at home, and religious practice.

ABCDE is the emergency framework — airway, breathing, circulation, disability, exposure — and it is used whenever a patient is acutely unwell, in that order, treating what you find before moving on.

The chain runs diagnosis → outcome → intervention, and it is what allows one nurse's plan to be read and continued by another, audited across a unit, and aggregated for research. Standardised language is also what makes nursing work visible in an electronic record where only coded data can be counted.

  • The goal shifts to the GREATEST NUMBER OF SURVIVORS overall
  • Which means the sickest may NOT go firstما يعني أن الأشدّ حالة قد لا يُقدَّم أولاً
  • This inversion is the point to remember

YELLOW — delayedserious injuries that can wait a short timeأصفر مؤجّل — إصابات خطيرة تحتمل انتظاراً قصيراً
GREEN — minorthe walking wounded
BLACK — expectantinjuries not survivable with the resources available
•Two things follow that surprise peopleيترتّب على ذلك أمران يفاجئان الناس
The patient in CARDIAC ARREST is NOT the first priorityIn a mass-casualty incident — because resuscitating one consumes the team that could save several. This is the counter-intuitive part of disaster triage.
And triage is REPEATEDCategories CHANGE as patients deteriorate or improve — a yellow can become a red, and a red can become a green after a simple intervention. It is a process, not a label applied once at the door.

The colour categories: red — immediate, life-threatening but survivable with prompt intervention; yellow — delayed, serious injuries that can wait a short time; green — minor, the walking wounded; black — expectant, injuries not survivable with the resources available, who receive comfort care.

Two things follow that surprise people. The patient in cardiac arrest in a mass-casualty incident is not the first priority, because resuscitating one consumes the team that could save several. And triage is repeated — categories change as patients deteriorate or improve, so a yellow reassessed twenty minutes later may now be red.

🚩
Watch out
Do not carry disaster logic into an ordinary shift. On a normal ward with normal resources, the sickest patient is still seen first.

2.20 · Transfer, Discharge and Medication Reconciliation

MEDICATION RECONCILIATION compares what the patient was ACTUALLY TAKING against what is NOW PRESCRIBED — at every transition: admission, transfer between units, and discharge.

1
A home medicine OMITTED
Stopped without anyone deciding to stop it — the commonest of the four.
2
A hospital medicine CONTINUED that should have stopped
A short course that became permanent.
3
A DUPLICATE
The same drug under two names, or two drugs of the same class.
4
And a WRONG DOSE or frequency
Carried across a transition without being checked.
5
So take the history from more than one source
The patient, his own boxes, the family, the community pharmacy and the primary care record — because the list in the notes is frequently not the list he takes.
•Transfer and discharge — where information is lost
A structured TRANSFER handoverCovers the same ground as a shift handover PLUS what is specific to the move: WHY he is being transferred, what is PENDING, what LINES and DEVICES are in place, and who to contact.
DISCHARGE teaching starts at ADMISSIONNot on the last morning — which is the single most consequential sentence in this section.
It coversThe medicines and WHAT CHANGED; warning signs and EXACTLY who to call; follow-up appointments; activity and wound care.
And TEACH-BACK — the check that the teaching landedAsk him to explain it back IN HIS OWN WORDS — ‘do you understand?’ is not teach-back, because everyone says yes.

Transitions are where information is lost, so a structured transfer handover covers the same ground as a shift handover plus what is specific to the move: why the patient is being transferred, what is pending, what lines and devices are in place, and what the receiving unit must watch for in the first hour.

Discharge teaching starts at admission, not on the last morning. It covers the medicines and what changed, warning signs and exactly who to call, follow-up appointments, activity and wound care, and — to check understanding — teach-back: asking the patient to explain the plan in his own words, which is the only way to know the teaching landed.

Barcode medication administration checks the patient's wristband against the drug at the moment of giving. It is a strong control and it fails in predictable ways: scanning the chart instead of the wristband, scanning a band that has been cut off and left on the table, or overriding the scan because the reader is slow. Every override should be a decision, not a habit.

Every system has downtime, planned or not. The unit's downtime procedure — paper forms, how orders are taken and verified, and how the records are entered afterwards — is something to know before it happens, not to work out at three in the morning while the system is dark.

In the Kingdom, quality and safety are enforced through the Saudi Central Board for Accreditation of Healthcare Institutions. Its Essential Safety Requirements are the minimum mandatory patient-safety standards a hospital must meet to be eligible at all.

ROOT CAUSE ANALYSIS looks for the SYSTEM CONDITIONS that let an error happen — look-alike packaging, a staffing gap, an ambiguous protocol — rather than the person at the end of the chain. It asks WHY repeatedly until it reaches something that can be CHANGED, and an analysis that ends at ‘the nurse was careless’ has stopped too early.

•JUST CULTURE distinguishes THREE things
HUMAN ERROR — a slipCONSOLED, and the SYSTEM is fixed. He did not choose it; punishing it only stops the reporting.
AT-RISK behaviour — drifting into a shortcutWhose danger has FADED FROM VIEW. COACHED — and the incentive behind the drift is removed, because the shortcut existed for a reason.
RECKLESS behaviour — conscious disregard of a known riskThis is the one that is SANCTIONED — and it is the only one of the three that is.
Why the distinction existsA culture that punishes all three equally gets NO reports and learns NOTHING — and the next patient meets the same system unchanged.

Just culture distinguishes three things. Human error — a slip — is consoled and the system is fixed. At-risk behaviour — drifting into a shortcut whose danger has faded from view — is coached, and the incentive behind the drift is removed. Reckless behaviour — knowingly disregarding a substantial risk — is still disciplined. Just culture is not a promise that nothing is ever a disciplinary matter.

Near misses are the cheapest data a hospital ever gets: the harm has not happened yet, and the system fault is already visible. Report them.
