1.1 · Leadership and Management Are Not the Same Thing
The distinction is the foundation of this whole part and it is examined constantly.
- Moves others towards a shared goal
- Sets a direction, inspires, develops people
- Does not require a title
- Comes from credibility
- Asks: where are we going?
- Comes with a formal position
- Plans, organises, staffs, directs, controls
- Accountable for budgets, rotas and resources
- Comes from the organisation
- Asks: are we doing it properly?
Power is where that credibility comes from, and the sources are worth knowing by name, because questions ask which one a described nurse is using.
The practical point: the two a clinical nurse has in abundance are EXPERT and REFERENT — and they are also the two that produce lasting change. A nurse trusted because she is good at her job and respected for how she treats people changes practice on a ward without any authority at all — which is exactly why the first consequence above matters.
1.2 · The Styles, and Choosing Between Them١٫٢ · الأنماط والاختيار بينها
- Directs without consultation
- Correct in an EMERGENCY — a cardiac arrest needs one clear voice
- And with a new or unsafe practitioner who needs direction
- Wrong as a default — it stops people thinking and stops them speaking up
- Gives autonomy, intervenes little
- Suits experienced, competent, self-directed practitioners
- Fails a new or struggling team — where it becomes absence, not autonomy
- The question always turns on who the team is
- Involves the team in the decision
- Better engagement and better decisions
- But it is slower
- Wrong in an emergency
- Inspires with a shared vision
- Develops people professionally
- Encourages innovation and challenge
- Attends to them individually
- Associated with better retention, satisfaction and patient outcomes
Situational leadership resolves the whole argument by saying that no style is right in itself — the style adapts to the readiness of the staff member FOR THE SPECIFIC TASK.
1.3 · Emotional Intelligence and What Actually Motivates Staff
Emotional intelligence is the capacity to recognise and manage emotion in yourself and in others, and it has five components examined by name.
Its absence is visible on a ward long before anyone names it.
What motivates nursing staff is not what managers usually assume, and the useful frame separates two kinds of factor.
- Pay, staffing levels, the rota, physical conditions, job security
- Their absence causes dissatisfaction
- But their presence does not create satisfaction
- Fixing them stops people being unhappy
- It does not make them motivated
- Achievement, recognition, the work itself, responsibility, advancement, growth
- These are what create satisfaction
- And they are largely free
- Which is why a well-paid nurse can still leave
- And a poorly-resourced ward can still retain people
So a manager who responds to poor morale only by trying to fix the rota is working on the wrong half — and one who offers recognition and development in a unit with dangerous staffing is working on the wrong half too. Both halves are needed, and they are not interchangeable.
1.4 · Conflict, the General Management Functions, and Looking After Yourself
Conflict is normal in any team doing difficult work under pressure, and the skill is choosing how to handle it rather than hoping it resolves. Five styles are examined by name.
- The issue is genuinely trivial
- Emotions are too high to discuss it usefully right now
- You need more information first
- Someone better placed should handle it
- It is temporary and CHOSEN
- The issue is important and keeps recurring
- It concerns patient safety
- You are avoiding it because it is uncomfortable
- Nothing is ever the right moment
- It is permanent and DEFAULT
The distinction is whether it is temporary and chosen, or permanent and default — and a leader who avoids every difficult conversation has not avoided conflict, only moved it onto the team.
Burnout is an occupational phenomenon rather than a personal weakness, with three recognisable components.
Note the second component especially: a nurse becoming cynical about patients is not a character change — it is a symptom. And because it is occupational, the response is addressing workload, staffing, control over the work, and support — not resilience training for the individual, which places the problem in the wrong place. Moral distress, from Chapter 04, feeds directly into it: knowing the right action and being prevented from taking it, repeatedly, is one of the routes into burnout.
1.5 · Saying the Difficult Thing
Feedback that changes behaviour has a shape.
- Privately, promptly, specifically
- Describe the behaviour and its effect
- “The dose was not double-checked, and that is how a tenfold error reaches a patient”
- Ask what happened from her side — there may be a reason you do not know
- Agree what changes, and follow it up
- In front of others
- About her character — “you are careless”
- Weeks later, in a general way
- As a sandwich that buries the message so well she leaves not knowing there was one
- With no agreement about what happens next
Receiving a concern from a junior is the harder skill and the more consequential. The reflex — to defend the plan, explain why the concern is unnecessary, or note how busy you are — teaches that raising things is not welcome. So thank her, take it seriously, and say what you are going to do about it — even when she was wrong, and especially then.
Speaking up to someone senior is where most preventable harm is stopped or not stopped, and it is harder in a hierarchical culture.
Finally, the local setting, which shapes all of this. Practice is regulated by the Saudi Commission for Health Specialties, which licenses nurses, classifies and registers practitioners, and sets the continuing professional development required for renewal — and facilities are accredited by CBAHI. SCFHS licenses the individual; CBAHI accredits the facility, and that pairing is worth holding.
1.6 · How the Work Is Organised
The model of care delivery a unit uses shapes everything about accountability, and four are examined.
Shared governance is the structure that gives staff nurses a formal voice in decisions about their own practice — councils or committees where bedside nurses, not only managers, decide on standards, practice changes and the work environment. It matters because it converts ‘we told them’ into ‘they decided’, and a change decided by the people who have to do it is the one that survives.
Interprofessional working is where most of the ward’s decisions actually get made, and the nurse’s position in it is distinctive.
Finally, the distinction that underlies this whole part and that the next chapter builds on: responsibility can be delegated; accountability cannot. A registered nurse may hand a task to someone else, and she remains answerable for the decision to delegate it, for choosing that person, for the instruction she gave, for the supervision, and for the outcome. ‘I told the assistant to do it’ is not a defence — and everything in Chapter 02 follows from that single sentence.
